The Care Manager will ask detailed questions to identify where your loved one could use extra support while recognizing the areas of daily life they manage independently.
Next, the Care Manager will assess essential Activities of Daily Living, such as bathing, dressing, cooking, and moving in and out of a chair or bed, to determine whether your loved one can manage independently or needs assistance.
After assessing the situation, the Care Manager will recommend care options tailored to the family's needs, addressing any identified gaps or challenges.
A home tour will help identify potential safety hazards, such as trip risks, missing grab bars in wet areas, or unsafe use of extension cords and space heaters.
The Care Manager will then review health concerns, medication management, and other key factors to incorporate into a comprehensive care coordination plan.
